Stem cell training in California

Next week I go to California for some training in clinical stem cell treatments. We are setting up a stem cell lab here under an Institutional Research Review Board which I have been working on for some time. It is all based through a network of doctors in which I participate. We are going to follow strict protocols and record a lot of data.

Is it a stemcellrevolution coming ?

This network of Doctors have  expertise in stem cell technology something patients until this time have had to travel far to obtain. So we will we bringing  this network into Hawaii over the next few months. This technology is on the cutting edge of regenerative medicine.  As far as I know we are the first ones to do this here clinically,  though there has been quite a lot of basic research activity. This has been with the medical school and with such  a company I have worked with in the past Tissue Genesis incorporated. They  recently left my building for a new office. We provided fat samples for years to help them advance their techniques and studies. This was more for basic scientific research rather than the clinical patient treatment trials that I am participating in and will be providing. They are one of the many  innovative companies which develop equipment and uses for stem cells.

We are primarily interested in use in cosmetic procedures and have had some experience with that previously,  but now we are formalizing this under a clinical research protocol in which patients can participate. There will be other physicians I am recruiting and getting involved here  such as neurosurgery and anesthesiology as  well as other specialties needed here in Hawaii.

The lab is being set up in my present facility. The equipment arrived two weeks ago and we are making room and organizing this lab.

So what will we treat and how? Well there are many conditions which show promise with stem cell therapy. To list a few :

Not embryonic cells so not controversial  

It is important to note that these are not embryonic stem cells,  which we will not use and there are ethical and very controversial issues which arise. These stem cells are harvested from the patients on fat which I have used on many occasions in the past.

The down side of this is that there have been many claims made by less than ethical health care providers to early on which basically made all types of claims taking advantage of some people with very sad disorders and through the internet have promoted these unfounded claims. So beware of something which sounds too good to be true. Although we have confidence that cellular therapies show great promise we are certainly not at the point we can promise unbelievable results. As far as that goes we never promise any results in any medical treatment  as it is considered unethical.

Laser Face Lift LAZERLIFT New Procedure with Dr. Pasquale Hawaii

LAZERLIFT -The true LASER face lift

There is a new procedure available in our office which is not the recycling of what has been touted before as a LASER facelift. This new technology is innovative and quite a new twist which has been tried in the past. It is truly a new type of LASER.

How LASERS tighten skin

First let me explain how LASER or non surgical technology in the past have tightened skin rather then the traditional way . The traditional way is when the surgeon cuts away the excess and pulls the skin to the new location such as in a surgical face lift.

With non surgical methods changes are made in the connective tissue typically by heat (LASER)  or radiofrequency waves ( Thermage) which makes changes in the connective tissues such collagen and “shrinking it” . This is only partially effective as physically removing the skin. So no one should expect the same results of a surgical lift, yet depending on the technology  used the non surgical skin tightening will shrink various areas and to various degrees of shrinking.

The Co2 LASER was the first to accomplish this by focusing a LASER beam on the surface of the skin heating it and causing the skin to shrink over time as it healed. We had and have one of these LASER’s known as Coherant Ultrapulse 5000c. A wonderful machine that did the best job at the time. Yet there were a lot of drawbacks such as the face looking quite bad for two weeks and persistent red to pink color and of course swelling. In Asians it is complicated because of long term hyper-pigmenatation (dark color). Yet for the time it worked ok.

There were variations developed which tried to limit the immediate healing time and prevent hyper-pigmentation, these were such LASERS as YAG and Erbium and the like and later a fractional LASER which skipped areas so that it was not as destructive. Yet these newer LASERS were not as effective so essentially one limited healing time yet also limited the effectiveness. This sorta was no pain no gain.

NEW way to tighten Skin

Well the new LASER system which is marketed as LAZERLIFT overcomes these external LASER’s by placing the LASER beam inside and under the skin. This basically heats the tissue from the inside so one does not see the same issues that one would see by heating the external skin surface. In fact by bring the LASER beam inside you can increase the effects of skin tightening by heating twice as much surface area. You can LASER the under surface of the skin and by turning the LASER in the opposite direction while inside towards the face one can change another layer of the facial tissue known as the SMAS which is layer above the facial muscles. This is the beauty of the technology, increased area of tissue changes and since the LASER is inside the tissue not LASER-ing the outer visitable skin layer.

The outer layer known as the epidermis and superficial dermis is not being hit with the LASER beam,  the problem with long healing time that one can visibly seen is gone and most likely any problem with hyper-pigmentation. Though healing is taking place its deep inside the face and not on the superficial skin visible to the world.

A truly wonderful new technology powering this LASER face lift with lessened healing time and good results.

My technique for Breast Augmentation very finely tuned after 25 years

All in all I guess I have been doing breast implants in Hawaii  for woman maybe 25 years or more. In that time I have seen it all I suppose. Such things as  belly button approach, trans axillary with an endoscope, infra mammary approach , peri areola approach , armpit approach , peri thelial approach. I have used every technique and have even invented my own technique and instrument. So,,,,,,,,,it goes.

In the years of doing breast augmentation I have finally settled into my way of doing this operation. Yet more important than actual technique, is understanding what the patient wants in terms of size of shape. This perhaps is one of the more difficult aspects of this surgery.That is to determine ahead of time what will make the patient happy.

Natural looking breast

As example,  I have many patients tell me “Oh Dr. Pasquale I want it to look really really natural” and then show me a picture they like of a woman who has super sized breast sticking straight out high on the chest and then tell me “Just like her”.  To my mind this is about as far away from “natural ” that I can imagine. So right there is a key, making sure that her “natural “is the same as your “natural” and indeed I encourage patients to bring or show me somehow what others look like that they consider natural.

I like to use the internet in my office and from my web site there is a large selection to show the patient as well as books of before and afters in my office we can show them. I watch the eyes of the patient and the body language to assess the desire of what she thinks.

Outside influence

Many times for example a patient will bring their mother or spouse and I have to look at the influence of that person on the patient. Sometimes for example a mother will not want her daughter to have a certain size and though the patient wants another size than the spouse or mother I have to navigate that one. So all of these factors are important.
